I had the same problem as Jim, so I drilled a hole in my single tank
adapter to compensate too.  I was affraid of it compromizing the strength
of the single tank adapter, but I haven't had any problems.

Jim, if you drill the hole diagonally, wouldn't the washer be less
effective, as it's no longer perpendicular to the direction the lead would
fall out?  I dunno...
