Problem is that this is not so much stupidity as lax thinking. Safe tech diving requires considerable compulsiveness- do everything the same way every time and don't make changes as you go along. There is also the factor of "familiarity breeds contempt"- thinking I've done this a dozen times with no problems, I don't have to follow ALL the rules, I'll remember that I didn't do X.
