My own opinion is that both the California and Florida was not a failure of 
training, instructors, or the training agencies, it was a failure of the 
divers to apply the training they had.

No instructor or agency would ever say it was safe to dive past 200' on air 
and to go past 300' on air is beyond my comprehension. I know that there are 
some that will for the "Buzz" (shade of the 'Call of the Wa Wa' ) but there 
are also some who put a needle in their arms for the same reason. In both 
cases you can't help the person if they do not want help.

In the case of the deep California death, I hear the first  comments of "Who 
was the instructor" and "I bet I can guess what agency the got their cert 
from" what does it matter which agency or instructor - they were stupid and 
disregarded what they may have been taught. What would you say if someone had 
a GUE cert and did something as stupid as this?

Some people can be instructed all day long every day and will still disregard 
what they have learned.
