The bubbles in the aerated saline are what the cardiologist sees moving from left to right if there is a PFO or other A-V defect (they're highly reflective on the echo). Without them, you can't visualize the defect. Based on my understanding of the process, as explained by my own cardiologist, there's no way the test you described could be useful.
