I've seen it happen a few times, both at the shop and on boats. I try and make it a point to check out my buddy's regs, both on descent and hang, for errant bubbles (though the dry bleed on Sherwood regs makes this a bit tougher...), and I check the hoses on my own regs post dive when I clean my stuff (btw, personally I've found the various hose wraps make it harder to spot problems and don't do a whole lot to prevent 'em). I'm pretty sure it's a common problem, but I think it's probably worse in up here in cold water where the temp stiffens the hoses and tends to work the swedge (where the metal meets the hose) a bit more.
