I've seen it happen on an HP line. A diver was showing off his super
all-singing digital Uwatec console (about the same size as my car), turned
on his air to show it working and the whole thing blew off and sailed
through the air like a mortar shell and plopped, by a lucky chance,
straight into 100m+ water.
