Sounds like the second stage hose failed (where the swivel connector is 
crimped onto the hose), not the regulator. I've seen it happen before 
(fortunately at the surface) when bottles are turned on and that first 
blast of gas pressurizes the line.  What was left on the end of the hose? 
 

Glad to hear that you're both OK. Just another reminder to everyone out 
there to regularly inspect hoses and equipment before planning long deco 
dives.
