Tha fallacy in this argument is that if you don't stick your head out they can't shoot you. To put it another way, I won't be on a dive where an instructor can "kill" me. I won't be so far over-extended that the instructor is worth that much more to my dive plan than a simple "buddy". So it might be possible to say my buddy was unable or unwilling to save me, but it will have little to do with whether there is any training going on with the dive. I find it much simpler to protect my own ass rather than to ferret out every instructor who may not be stellar. Its a lot more gratifying to start a jihad, but its a job that will never be complete. In the meantime, hell I'll just be diving, prepared to survive an encounter no matter how poor the instructor. And I'm not trying to be selfish, I share the advice freely. Cover your own ass. Its the best protection against anything, bad instructors included. You'd be suprised how hard it is to "kill" someone when they don't make it easy for you.
