Jim, if you use Liquid Nails to attch the mouthpiece, how do you attach
the neck strap (shock cord or surgical tubing) to the backup reg?

I'm guessing you must insert the mouthpiece into a loop in the shock
cord, which allows the reg to be pulled free. Everyone I've ever seen
using this approach formed the loop with... you guessed it, a pair of
zip ties. It could be done with knots, I suspect, but regardless I
don't think it's the best approach. I don't want the regulator going
anywhere without permission, and a single zip tie securing a permanent
neckstrap is the best way to do that that I know of. They don't
exhibit the twist-away weakness in this context.

p.s. Has anyone tried eliminating the backup reg's mouthpiece and
neck strap altogether, by getting a tracheotomy and inserting
the 2nd stage directly into the stoma? Perhaps one of the
smokers out there could let us all know how well this works.
