Terry,

It does make one wonder.  

However, it is hard to understand a reason for switching gases at max depth,
so I have to wonder about equipment failure, and that Ed might have had
to switch to air because of this????

Many tech divers carry their gas supply as independents, and many do not
use redundant regulaors on these independent bottles.  Thus, if the bottom
mix is carried in a single bottle (air, or some other intermediate mix
is used down to a target depth, and then switch to mix for the deeper
phase of the dive), the loss of this mix in an overhead environment
would necessitate switching to air or the intermediate mix.  Obviously,
if the failure was a blown burst disk or tank neck oring, then there is no
real alternative other than switching to air or intermediate mix.  However,
if the regulator failed, then having a Y-valve on that tank with an
additional first stage would be the ideal way to go. 

Will be looking forward to hearding more about this accident, and the
equipment configuration in use.
