Have you ever had a backup reg come loose from the holder?

I have used mine for about 200 dives and it has NEVER come loose.
Not even close. I use a tight loop in the tubing so that I can pull it away 
if  necessary, but requires a strong pull.
This is one of those things I consider a non-credible failure. If you
make the loop correctly, the reg does not fall out.
I am not suggesting that you try this (I know you will not, as you have
your paranoia points) but it does work. If it did not work for you, then 
you may not have fashioned it correctly.
