Dave,

No, the surgical tubing is not tied "around your neck". I said "the 
backup regulator hangs around your neck held on by a piece of 
surgical tubing". Make a loop of surgical tubing that is attached to 
your mouthpiece. The loop should be big enough that your head fits 
through comfortably. Viola. It's demonstrated in the video.

Gravity keeps it positioned correctly.
