How about this sportdiving example?

2nd stage octo stuffed into bc pocket that was securely velcro'd/zippered 
shut.  This configuration was reportedly sighted on a local diver who's buddy 
died on the same dive in an out-of-air emergency.  

According to reports the out-of-air diver, when unable to retrieve the octo, 
tore away the person's primary reg knocking their mask off in the process.  
The buddy with no mask or regulator apparently panicked and bolted for the 
surface (from approx 60') leaving the out-of-air diver behind to drown. -KL
