I posed a variation of this question to the list, awhile ago.  My question was, 
(sic): 

Does anyone have first hand knowledge of a non O2 cleaned regulator suffering a 
catastrophic failure, when used with pure O2?  I'm not referring to  O rings and
the HP seat merely wearing out in one year on O2, as opposed to 4 or 5 years (or
whatever) on air.  I'm talking about a catastrophic failure, i.e... fire or 
explosion.  

Although my question prompted a good deal of discussion, nobody ever reported a 
catastrophic failure due to the PP O2.

How many people have...seen a... SCUBA tank explode when mixing nitrox 
because they 
had air put in them after they had been O2 cleaned?
