I for one never have, with my experience being certified nitrox for 6 months and hanging out on many of the net scuba forums for several years. I have heard people relate stories of accidents in home-brewing, but I have personally never heard of any problem occurring in a shop. All this being said (that I personally have not heard of a story which would cause caution to be used) I do err on the side of caution b/c I simply don't have a good personal feel for how/not explosive O2 is.
