Subject: Exploding unclean nitrox tanks

I figured the subject line would draw attention; this is a serious 
question.

After reading this "to put air in my O2 clean tanks/to not put air in 
my O2 clean tanks" I have a question.

How many people have heard of, seen, read an article about, knew a guy
who had his SCUBA tanks explode when mixing nitrox because they 
had air put in them after they had been O2 cleaned?
