While nobody should ever overpressurize tanks due to the risk involved (104's cranked to 4000 PSI weigh so much you risk a hernia just lifting them from the fill tank, and God knows how much trouble you can get into with 360+ ft3 of air on your back!), it does seem to be fairly common in the cave community. I have heard that certain tanks, e.g., 104s, were designed with a very large safety margin (in terms of rated pressure vs. failure pressure), whereas others, e.g., Al 80s, have a significantly smaller safety margin.
