Well, explosive decompression is the real problem in my mind -- and I
don't think it is all that rare (it happened to me once, though many
years ago).  In the old days, an explosive decompression incident on a
commercial airliner put you at about 30 000 ft; today it may well put
you at 50 000 ft -- I would not want to take a serious DCI hit hours
away from help (though O2 is readily available :-) just because of
"technical difficulties"!
