There's always an element of apparent randomness to O-ring extrusion -
temperature is often a unappreciated factor, for example.

Can't answer all you questions, but it is worth mentioning that tank neck
O-rings should ideally be harder, less extrudable material than standard
generic 70 duro O-rings, say 90 duro, and should never be greased.
