JT,
I knew you would come up with this, and you are right in most aspects,
especially when writing each case is different.

Of course, there are undeserved hits, like material failure ( one guy
had a broken seam on his drysuit boot in cold water), or things
developing to the worse. A diver may panick because of feeling lost, get
seasick, and else. Had that times and again. 
Mostly unexperienced divers get lost underwater, and try a free ascent.
But quite a few instructors got their marks on owing a bottle of
calvados to the boat crew for being fetched by chase boat taxi.
The worst case I experienced was when a group of 3, a CMAS ***
instructor, his buddy and a 65 year old ex -Instructor got up behind. In
their efforts to reach the boat upcurrent, they left the old one behind.
We were ready to act, and the old guy was near panic, at least deeply
exasperated, when I arrived with the rib.
