William, having only used the reel a few times, and being generally a novice at this, I have had this problem. However, I always know when it's going to happen. It's when the reel wasn't wound tightly. I haven't had it happen lately, except when I let someone else reel back (for practice). I find that running a reel takes a great deal of concentration, especially if I'm holding a light, using dry gloves, and keeping track of my buddy. Basically, the dive changes from a sightseeing dive to more of a work dive. If I pay less attention, I will invariably drift forward, release the tension on the line, and then the reel won't be wound properly and the line might "fall off" later. The "falling off" problem could be solved by shortening the line. If you had a little bit more reel left above the spooled line, it wouldn't fall off so easily. FWIW, I've never had the set screw come loose on me, but I'm pretty thorough when tightening it.
