As for Sheck breaking rules, didn't he break just about all of them, including his own "Don't dive deep in caves." Regarding the Zacaton dive in 1994, it is my understanding that Exley and Bowden were within sight of each other for most of the descent. At 925 feet, Bowden could no longer see Sheck who was below him, and began his ascent solo. Exley's body surfaced three days later.
