Karen - I've had a quick release on a weight belt fail. I felt
the belt slide and grabbed it as it dropped.

My son had his belt slide down when the quick release let go. 

Other divers I know of, at all different levels, have had quick
releases fail or accidently knocked them open causing the belt
to either fall off or slide loose.

The other kind of quick releases (like fin strap buckles, only
bigger) are also failure prone. I have broken one of the tongues
off one. For that matter, I have also broken a fin strap buckle.

I'm sure, if you ask around, you will find other divers who have
had similar experiences.

Now you have heard of a quick release failing in the field.
