Unfortunately these incidents happen all too often,
and
even with good dive planning.  Nearly every time I
have
been on a charter boat (Great Lakes) with technical
divers on board, I have seen some type of incident
like
the one described.  It is because of the "dufus
factor"
(I believe you call them "strokes") that I prefer to
dive from my own boat, with my own buddies, preferably
on a wreck that has no other boats tied up to it at
the
same time.  May good luck be with us all.
