You must never hoseclamp a d-ring to a tank, because this produces a
metal-to-metal connection that can not be cut loose if you become
entangled.  In DIR, the only D-rings you have are one on each side of
the chest and one on the left hip.  You can make it easier to clip off
to the chest rings by bending them outward slightly.  All cylinder
attachments are accomplished by bolt snaps attached to the cylinder. 
It sounds like you haven't seen the DIR II tape - this goes over all of
this, and is well worth the cost.  Call Halcyon or Extreme-Exposure for
a copy.  I dive cold water exclusively, and don't have any problem with
manipulating bolt snaps.  You just need ones that are large enough, and
preferably with square thumb slides instead of rounded ones.
