I once got it before the first deep-stop in around 80m (245ft).
It was a shitty feeling.
The next day i developed a cold, so i think it was related to that.
Normally i also get it on shallow dives, often in pool lessons.
My doc checked everything but did not find anything.
If possible i try to continue ascent. When i get disoriented i just stop
and
wait for a few seconds until my ears equalize.
I stick to my bottom timer during the ascent to watch my ascent rates,
cause
i dont trust myself then.... ;-)
