Dan On the hydro as stated the instructor in charge was Jim Mims not Derrick . Can you read Jim Mims was the instructor responsible for the students on the dive although it was Derricks course-- Read the post. On the OW dives these were not IANTD courses and they were Dan Mitchell's students and Dan was the PDIC instructor in charge of that PDIC course Below is a copy straight from our QA manual and it is being followed to the T with all the dots periods and detail imaginable I have just sent Carl a unsubscribe post as I will be out of town teaching a cave course so will not be able to respond to whatever you post nor do I intend to respond on this issue anymore. For those interested IANTD is following its detailed QA procedure spelled out. This procedure was developed to ensure a fair decision on behalf of all parties in the event of situations such as this one. Direct from the IANTD QA manual this has all been completed and the package is being sent to the BOA-QA board they have all the required info and a lot of it is much different than reported on this forum not better or worse but different. The BOA will have sufficient data to make a just decision we have even sent all of George's and Dans comments along with the package so nothing is hidden distorted or left out I do not endorse or condemn Derrick that is the decision of the BOA QA board not mine and not Dans or George's on behalf of IANTD Dan if you want to punch Derrick out that is your decision, On all OW IANTD training dives with Derricks courses another instructor will be in chargé until the investigation is complete, just as it was on the Hydro with Jim Mims being in charge and yes Derrick can dive with them in fact as they are his students most will feel more comfortable with him along even though the other instructor is in charge as they know him and have already been trained up to this point by him. So his presence in the water is for a purpose even if he is not in charge of the dive. It also provides the instructor in charge a chance to evaluate the dive planning etc pre and post dive. We feel it is a good process Tom .. IANTD Process When a Diving Fatality Occurs During Training IANTD will immediately contact the Instructor and obtain an Incident Report. This will be followed up by a more comprehensive report, which includes obtaining all information pertinent to the accident, statements from Students Other divers on the dive A QA form being sent to all former students of the instructor. Determinations by the ME(Medical Examiner),if available Reports form the recovery team, Reports from the diving vessel Review of the training program conducted to the date of the accident, Statements from other sources who are aware of the diving instructor and his practices Statements from others who are familiar with the capabilities of the accident victim These include people who have dived with or observed the instructor and student during training. Once all this information is compiled the data is sent to a QA board consisting of members of the IANTD international BOA. Most of these people will not even know a given instructor and will be totally impartial in reviewing all of the data. First immediate step: Limited teaching status 1. The instructor must have another instructor present during all OW or Overhead environment dives. The instructor may conduct all lectures and confined water sessions without a second instructor present Based on the BOA determinations one of the following will take place. These actions are for the overall protection of the instructor; the public and IANTD while ensuring fair and just due process. Events following an investigation 1. If it is found that the instructor is not at fault in the accident in any way whatsoever then the limited teaching status will be removed and the instructor will go back on normal teaching status. 2. If the BOA determines that the events are questionable but not directly controllable or preventable by the instructor The instructor will be placed on probational or restricted statues and the BOA will spell out specific procedures to be adhered to in order to be reinstated to normal teaching status. 3. If the BOA finds that the instructor did not act responsibility the instructor may be suspended with the ability to request an appeal to the BOA. A suspended instructor may or may not be given a provision for reinstatement following one year of suspension again as determined by the BOA based on its finding of all the facts. 4.
