You would think that the physicals required for him as a pilot would 
appropriately cover all issues, especially since he had been technical 
diving for quite some time now.

Stress is an issue when doing many thing at we do as adults. This include 
pushing lawn mowers, hauling dive gear, diving, water sking, sport fishing 
(check the stats on those numbers), etc.

The medical examiner's report said that Tai Wilkerson suffered from 80%
blockage of his main arteries. He was a walking heart attack, and the
stress of this dive was enough to push him over the edge.

I think the lesson here is to have a stress test and/or ultrasonic scan
of your carotid arteries before taking on tech diving or any other
strenuous sport.
