Lee,

I have never had it occur while diving but I have had the burst disc let
loose on two different valves.  One valve's disc released on two
different occasions (10 years appart). In both cases the tank was in
storage for the off season in the house.  On neither occasion was the
tank exposed to excessive heat.  With the second failure I replaced the
valve with another I had on hand not wishing to take a chance that there
was a bur or some other defect which caused the problem.   The other tank
 developed a slow leak in the disk ( about a week after it had last been
filled it had bled down to 1100 psi from 2250).  The disk has been
replaced and it has been fine ever since (a year).
