Happened to me, although not in a cave.

A person who I no longer dive with put the last of an empty tank into her 
drysuit as we dropped passed 80 feet, and then grabbed my tank valve in 
distress, and requested the reg in my mouth.

I use a Sea-cure custom mouthpiece, which is way bigger than standard 
pieces.

She reported no problems whatsoever.

She said that mouthpieces were definetly not the problem, and she could 
have found a way with any mouthpiece I had, as long as it delivered air.
