Yes.  I have.  I now consider myself very very lucky.  I don't know
that I'd assemble the same 100% O2 rig again.  I'd feel much more confortable
with cleaned equipment.

No.  Not yet.   But hopefully, I'm getting wiser (not stupider as some have 
suggested :-)).

P.S. I am also extremely leary (wary) of 100% O2 decompression, due to reports
    of this being hard on lung tissues.  I am much more comfortable with 
    high O2 Nitrox mixes, and assume air for deco tables/computer simplicity.
