I'm straining my brain, but I can't think of any specific instance. 
However, I think the lack of publicized incidents is more a reflection of
the very few number of individuals who have used non-cleaned scuba
regulators with O2.  Although non-cleaned regs. can and have been used for
O2, I wouldn't recommend it.  I HAVE heard of many instances where
supposedly clean systems have ignited (not SCUBA systems), so it is a
reality.  I'd hate to come up to my O2 stop, open the valve on my O2
bottle a bit too fast, and have the thing auto-ignite on me!  IMO, this
one isn't a chance worth taking.

P.S. I realize I'm not being P.C. by excitedly and hotly lambasting the
suggestion of non-cleaned O2 regs, but I'm more interested in
disseminating my honest opinions than I am in being P.C.
