I forgot:

1.5 The diver AND his buddy should have verified that their cylinders were 
full before entering the dive.  If it was to be suspected that the gauge was 
wrong and that the cylinder was full, this should have been verified with 
another gauge.  BTW, I'm sure that the diver was not alone... hey?
