Dave,
	I'm the one who mentioned it in passing.  No, I've never heard of
it happening.  It's the only "tank-level" failure I can conceive of that
should even be mentioned in a discussion of redundancy. (in other words,
carrying a second tank of a particular mixture to compensate for the
primary tank rupturing is likely a waste of time:  I seriously doubt that
a diver attached to a ruptured tank would live long enough to worry about
an alternate breathing source.)

IMO, the probability of a tank/valve o-ring failing during the course of a
dive is so tiny that I don't worry about trying to compensate for it when
thinking about components of dive equipment to back-up.
