Yeah, glad you brought it up, i almost forgot about both incidents...
in my limited experience, about twenty five years of diving, i have
personally seen two valve o rings pop while under pressure:

the first, a neck o ring, happened about ten years ago while filling a 
steel 72 in a vat of water: the o ring blew at about 2100psi, it took 
about five
seconds to empty the tank (and the vat).. i couldn't tell if my pants
were wet from the water or something else... the other people in the
shop wouldn't come back in even after i explained it wasn't my fault.

the other more recient, the orfice o ring, was in my shop one year ago, it 
was on a brand new
(one month) aluminum 50 that was filled to three thousand psi that i
had used for demo in an open water class. (i had forgotten to shut off
the valve after the demo)  the security company called
and woke me up at about 1am and said the alarm had gone off.. when i
got to the shop the tank was wet from condensation and the reg first
stage was loose... whew !! as you say... a new paranoia, every time i
pass the 300fsw mark i see that little wet tank and go over my manifold
switches in my head...

remember that the valve will still be blocking *most* of the hole,
but still, in my experience, it will only take about four or five
seconds for the tank to drain.. gulp..

 Are they routinely changed when a tank is

YES, when a vip is done properly both valve orings should be changed.
