Andy (and others) I know of a diver who has had a tank O ring blow in a cave. Fortunatley the leakage was not far back and he exited without incident. It can and has happened. Why? As with many others he has relied on the "highly experienced dive shop" to do annual VIP's etc. Though many are good, there are many with kids doing a summer job, etc. It is believed that regular VIP's did not always include replacement O rings as they should of. Suspected problem. Regardless, it is possible and has happened. For information purposes, he like I make sure our gear is in top condition by doing it ourselves. We know its done. I'm trained through PSI on visuals. A quick note: In the words or Richard Pyle (sort of<G>), you are the one who is responsible for you in the end. You have the most to loose and you are the most motivated. When it comes to your life, like packing your parachute, you once trained, will be the most motivated to be sure its right.
