Curt,

The chances I agree are very small, although I have seen many manifold/valve
failures, most of which were OMS (which I personally would not be caught
diving with) due to poor safety design (which I believe has been resolved by
OMS), and a sherwood due to the o-ring on the nipple deteriorating. All
happened at the time of filling the cylinders or on the dive boat prior to
entering the water.

But more of a probability then manifold failure would be a regulator problem,
specifically an uncontrollable free flow, which without an isolator would
 drain both tanks. The isolator valve in this situation would allow you of
course to  isolate the loss of gas to only the cylinder with the free flowing
reg.  attached to it.
