Bob, I do not know about "failures" per se, but I can tell you the reason I would not use an OMS manifold myself is the position of the o-rings. In my opinion, a manifold needs to have barrel rings, like the Scuba Pro and be adjustable latterally , like the Scuba Pro, not face seal o-rings. It needs to be counter-threaded like the scuba pro so that any movement of the center will result in a concurrent widening or narrowing of the width, rather than a loosening or tightening of the point where the o-rings seal. This is one of those things that would qualify for the comment, "some things are obvious, and some things are obviously wrong". In my not at all humble opinion, the OMS is a "wrong". Maybe they can join the other manufacturers and agencies and sue me - that way I can get all of these insects in one roach motel. By the way, I have discarded all of my other manifolds, and now use the 300 bar D.I.N. Sea Elite, which meets all of the requirements. It is available from Divers Supply in Gainseville, Fl.
