As Bernie Chowdhury pointed out it is difficult at best to obtain reliable information on diving fatalities. We wanted to include this information as part of a regular column in DeepTech Journal but found it difficult to be certain about the accuracy of the reports. And printing an inaccurate report is worse than not printing anything at all. One has only to look at the intense flame war that ignited when a member of the WKPP team recently died (in a non-WKPP dive). Unfortunately the truth is a relative thing. When two people witness an event they may have very different recollections of the specifics. There are also those who would protect the reputation of a friend by sanitizing the actual events in the case where the victim did something stupid and paid with his life. When fatalities occur during a training dive, as recently happened in a deep lake dive in Wisconsin, the instructor is prohibited from discussing the event by his associations laywers. When he finally does describe the events you can be sure the description is one that is favorable to him and his association (the insurance underwriters). Yes I believe that accident analysis is beneficial but I know of no way to ensure the accuracy of the events and thus the accuracy of the analysis.
