I am not a rebreather diver and I will not comment on the specifics of the
aforementioned accident, however, I believe Scott B. is right on here with
respect to "complacency".  I would only add that, from my experience in
observing other divers, the complacency (and ignorance) is not specific to
rebreather diving.  It seems these problems are prevalent in most forms of
diving.  The main difference may be rebreathers are, at this point, much less
forgiving.
