When I first started looking into tech diving incidents to see if some lessons could be learned, I was surprised by the stonewalling. I'm not surprised any more. Crap like this is the usual response to any reported problem, so very few people report anything that is less than perfect. It's a damn shame since under these conditions very little can be learned without an autopsy being involved.
