I'm afraid not. It all sounds so similar. We had a drowned Padi class
subscribent here in a quarry where some tec-divers use to train. He got
lost swimming into a submerged cellar at 33m, got lost in zero viz,
panicked  and drowned with 100 bar left in the tank.
The instructor told the examiner he swam away deliberately from the
group ( of at least four people, to many in this place). He was not !
punished.
