Charlie -

I know of only two actual failures in use -- the first led to the 
Halcyon/EE recall; the second was a failure of the same type of knob on a 
Diver's Supply/Sea Elite manifold which was recently reported on the 
Florida Techdiver list. 

The underlying problem is that the "fake" Sherwood-style knobs 
preinstalled on recent manifolds supplied to both Halcyon and Divers 
Supply are defectively designed.  Only a thin metal disk molded into the 
knob engages the valve stem instead of the large brass insert in the 
genuine Sherwood knobs.  Once the disk rips free -- and it's likely to 
over time -- you won't be able to turn the valve.  Is it an imminent 
safety hazard?  Probably not since the knobs are most likely going to 
fail when you turn them hard to a stop -- either all the way off or all 
the way on -- but DIR is all about eliminating unnecessary risks and a 
knob on the verge of failing completely could prevent you from isolating 
a valve in an emergency.  Even if not life-threatening, a manifold 
failure could certainly ruin a good day of diving.

Only Halcyon has stepped up to the plate and issued a recall, but since 
real Sherwood knobs and washers are only a few dollars and take seconds 
to install, there's no reason to take the risk with any of the manifolds.

BTW, I happen to have a Sea Elite manifold with the knock-off Sherwood 
knobs, so I've had a chance to take a close look at them.  I'll be 
replacing them this weekend!
