Here in Oz we had a double fatality quite a few years ago now when two DM's (FWTIW) penetrated the one place they shouldn't on the Himma, a small, as in tug size as I recall, wreck off the northern beaches of Sydney lying in 46m (151 ft) or so. This chamber is particularly dark, lots of nooks and crannies, lots of downhanging conduit and only one entry/exit which means lots of silt. They had absolutely no right to go in there. Dammit, they didn't even have a penetration line. Both were recovered with breathable quantities of air in their tanks. Whilst I certainly am not aware of the specifics of the coronial enquiry (Strike? BillyW?) I thought this odd at the time, considering they were trapped and therefore couldn't do anything other than panic. So did they panic enough that they forgot to breathe?
