Maybe I missed something in the report but it seems like they didn't have a proper bailout plan. How can you function (let alone try to decompress) with a 10% O2 mix. It seems like maybe an extra bottle of 50/50 may have made a difference? At least @ the 60' stop (where a problem seemed very apparent) they could have switched to the 50/50 and did the decompression on open circuit.
