Bill, for open water setups the hose is best under the arm since you otherwise would need some little mutant hose to make it not stick way out in the breeze. For the real deal, the offset regulator provides the perfect fit for a fair lead from the left reg to the mouth. Under the arm in this case will require a huge hose and if you break the bungee, you will not find that reg when you need it, not to mention that you also have you canister light on the side .
