The upside down stirring sign, as explained by Mr. Pelstring, is what is used to turn the dive when there are no aggravating circumstances provoking the "turn". Pointing to the SPG clarifies the reason for the turn. The rate of exit will be without undue urgency with this signal. If there is a need to exit the cave/wreck with a degree of urgency, or at least to avoid "sightseeing" on the way out, the thumb gets thrown. Throwing the thumb during exit after the dive is turned with the upside-down stirring signal conveys a need for a more urgent exit than what is currently taking place, a.k.a. "You're going too damn slow." It's all about being clear and concise.
