And the other point is that to say nothing so as to not offend the family members ONLY SERVES THE PURPOSE OF PROTECTING THOSE RESPONSIBLE. While it may be painful to realize that someone died doing things "by the book" or "by what they were taught", at least they can have the comfort of knowing that this brought about a change so that noone else will have to go through the same thing. It also makes for nice fuel for the lawsuits when they find out that the stupid thing their loved one did was exactly what they were taught. This is the real reason for the excuse of the "effect on the family members" line of reasoning.
