There was an article ten years ago in a British dive magazine that
showed some 96 out of 100 CNS hits on  sample of  divers using deco
computers who got bent were due to PFO. The other four were due to HFS.
At the time, interestingly enough, the term PFO was used in the article
with zero explanation, as if everyone knew what it was. I had to ask my
wife what it meant. The next day I got a test.

This all got swept under the rug and kept as quiet as possible for
years. Just think if Tom Mount had to require a PFO test for everyone
IANTD trains, for exanmple.
