Initially I was not going to cover this, since I assumed that it was a
cave
dive, and therefore outside the parameters that I have set for myself
(i.e.
technical dives excluding cave dives, since cave dives are reported
through
a good existing system.)

I would like to know the date this occurred (I assume Saturday
11/20/1999), the name of the victim, and the name of the instructor. 
(These are important for a number
of reasons, including correlating with other reports, verification, and
so on.)

Any other information would be helpful.
